Brent CCG is reviewing community palliative care services for patients in their last phase of life. End of Life Care (EOLC) services are provided by 4 different hospice providers. Recently, the in patient unit at Pembridge Hospice provided by the Central London Community Healthcare NHS Trust was closed.
We would like to hear from Brent patients and carers about their experiences of using local community palliative care services. These services include in-patient hospice care, hospice out-patients, Hospice at Home and day hospice and community specialist palliative care. Community specialist palliative care services are fragmented and there are variations in levels of quality, access and availability.
Brent CCG is working with providers to create more joined up care for patients.
