The NHS’s move towards integrated care systems offers Brent an excellent opportunity to use insight and feedback from patients and users and to co-ordinate that feedback across pathways of care.
NHS organisations and Brent Council are joining forces to integrate (or join up) services for Brent people. We will also invest in preventing Brent people becoming ill and keeping them out of hospital. Our aim is that people in Brent can live healthier lives and get the care and treatment they need, in the right place, at the right time, out of hospital as appropriate.
To make this happen, all parts of the local Brent system – such as GPs, care homes and home care, hospitals, community and mental health services – are working together more closely than ever before.
We have come together to form a local ‘integrated care partnership’ (ICP) overseen by a Board which includes patient representatives, Healthwatch and CVS Brent
